Loperamide HCl
Indications/Uses
Control & symptomatic relief of acute nonspecific diarrhea & chronic diarrhea associated w/ inflammatory bowel disease. Reduction of number & vol of discharge in patients w/ ileostomies & colostomies.
Dosage/Direction for Use
Adult & childn over 12 yr Symptomatic relief of acute diarrhea Initially 2 cap followed by 1 cap after each loose bowel movement. Max: 16 mg daily (8 cap). Discontinue use if there is no improvement after 2 days. Symptomatic relief of chronic diarrhea Initially 2 cap followed by 1 cap after each loose bowel movement, then reduce dose to meet individual requirements. When optimal daily dose has been established, administer as a single dose or in divided doses. Ave daily maintenance dose: 2-4 cap daily. If no clinical improvement is observed after treatment w/ 8 cap daily (16 mg daily) for at least 10 days, symptoms are unlikely to be controlled.
Administration
May be taken with or without food.
Contraindications
Hypersensitivity. Constipation, swollen abdomen or abdominal pain, acute dysentery characterized by blood in stools & high fever; pseudomembranous colitis, active ulcerative colitis, meningitis or history of liver disease. Pregnancy & lactation. Childn <12 yr.
Special Precautions
Appropriate fluid & electrolyte intake. Patients w/ AIDS should stop therapy at the earliest signs of abdominal distention. Toxic megacolon in AIDS pateints w/ infectious colitis. Loss/depressed level of consciousness, tiredness, dizziness, or drowsiness may occur. Avoid alcohol or milk, fruits, green vegetables, & spicy or fatty foods. Childn.
Adverse Reactions
Dry mouth, abdominal pain, distention or discomfort, nausea, vomiting, fatigue, flatulence, constipation, epigastric pain, drowsiness & dizziness. Hypersensitivity reactions eg, pruritus, rashes, urticaria. Rarely, anaphylactic shock, angioedema & bullous eruptions including SJS, erythema multiforme, & TEN; paralytic ileus, toxic megacolon, urinary retention, hypertonia, loss/depressed level of consciousness, coordination abnormality, & miosis.
Drug Interactions
Increased plasma levels w/ ritonavir, quinidine, oral desmopressin, itraconazole or ketoconazole, gemfibrozil. Increased constipation w/ drugs that slow down the action of stomach & intestines (eg, drugs for depression & cold & allergy medications). Drugs causing drowsiness.
